Comprehensive person-centered service plan sent to the state.
When the department is paying for a resident's services, the enhanced services facility must give the department case manager a copy of the comprehensive person-centered service plan each time it is completed or updated. The ESF must keep a copy of the person-centered service plan that has been signed and dated by the department case manager, in the resident's file.
