(1) The evaluation of contractor or design-builder performance shall be made on a form in the formats established by the department and published on the WSDOT website at https://wsdot.wa.gov/business-wsdot/how-do-business-us/electronic-forms.
(2) The Prime Contractor Performance Report Manual and the Design-Builder Performance Report Manual provide detailed instructions for preparation of the performance report.
