What if the companion home provider is no longer willing or able to provide services to a client?
(1) If a companion home provider is no longer willing or able to provide services to a client, they must provide a sixty-day written notice to:
(a) The client;
(b) The client's legal representative if the client has one; and
(c) DDA.
(2) If a companion home provider must terminate services immediately due to unforeseen circumstances, the provider must give a three business days' written notice to:
(a) The client;
(b) The client's legal representative if the client has one; and
(c) DDA.
