Application—Generally.

The applicant must send a completed application to the department for any of the following actions:

(1) To receive an initial adult family home license;

(2) To change ownership of the adult family home;

(3) To change the location or address of the adult family home; or

(4) To increase the licensed capacity of the adult family home.
