ECEAP contractors and providers must verify children's dates of birth for the purposes of enrollment using one of the following documents that includes their date of birth:
(1) A document listed in RCW 43.216.528;
(2) Certificate of Indian blood;
(3) Tribal enrollment card; or
(4) A document approved by the department.
