What documentation is required for successful completion of geriatric behavioral health worker training?

Geriatric behavioral health worker training must be documented by a certificate, transcript, or proof of successful completion of training issued by a qualified instructor that includes:
(1) The name of the student;
(2) The title of the training as approved by the department;
(3) The number of hours of the training;
(4) The name of the facility providing the training;
(5) The instructor's name;
(6) The instructor's signature or other authorized signature from the training entity; and
(7) The completion date of the training.
