Who is a qualified provider of therapeutic adaptations?

(1) A qualified provider of therapeutic adaptations installation is an entity who is contracted with DDA as an environmental adaptations vendor.

(2) A qualified provider of therapeutic adaptation items and supplies is an entity contracted with DDA as:

(a) A specialized equipment and supplies vendor;

(b) A purchasing goods and services vendor;

(d) An environmental adaptations vendor.
