(1) If you are manufacturing factory-built temporary worker housing structures at more than one location, approved design plans must be available at each manufacturing location.

(2) You are required to send us the following information for each manufacturing location:

(a) Company name;

(b) Mailing and physical address; and

(c) Phone and fax number if available.

(3) You must update this information as it changes.
